Currently I have Mac OS X Lion and Xcode 4.2 with the iOS 5 SDK. I also have an iPhone 4 with iOS 5 installed, but I want to run my app with the iOS 4.3.5 SDK.
Which Xcode version has iOS 4.3.5 SDK installed and which Mac OS version supports that version?

If you want to test your app on 4.3, launch XCode 4.2, go to preferences, then from the "Downloads" section choose to install the 4.3 Simulator.
I dont remember any xcode version with iOS SDK 4.3.5. Xcode 3.2.6 used to come with 4.3 SDK. In case you want to use 4.3 SDK, you can copy 4.3 SDK into the sdks directory in Xcode 4.2 itself and restart the xcode to use the 4.3 sdk.

Since downgrading the iOS version is difficult and doesn’t sound like an option for you, I think you must follow the other option:
update to the latest version of the iOS SDK
The iOS 6.1 SDK is included in Xcode 4.6.
Make your code compatible for ios 6.1
and try using xcode 4.5
I upgrade my iPhone OS 5 to OS 5.1.1, now my Xcode 4.2 can't identify the device (Mac OS is Lion 10.7.2).

You have a rather old version of Xcode. You should upgrade to Xcode 4.2 and get a new SDK. By default SDK 5 is used. However it is possible to download older SDKs.
Choose Xcode -> Preferences and then Downloads. Press the Componentsbutton and you will have access to support for older stuff. Select the components that you want to download in the list.
It is not possible to install the SDK separate from Xcode, according to this.
XCode => 3.2.6 should work. However, any XCode 4 version should work as well. You'll have to download the IOS4 SDK manually for those, as the current XCode Packages only include the IOS 5 SDK.

I am running XCode 3.2.3 with the iPhone 4 SDK. I have paid apple and enrolled, added my certificates and provisioning profiles. Now, I have plugged my iPod into Xcode and clicked "Use for development". My iPhone is running iOS 4.1 and Xcode says -

You have to update your XCode to latest version. Along with that iOS will be upgraded to 4.1 automatically.
